    Hi @crsmith58, welcome to our community and for asking your question.
    Something that I'm finding really odd, is that you're asking for your network name and password.  If you set up that device, you should know what that is.  However, even if that's true here is what I would recommend:
    1.  If you are leasing your router from your internet provider, contact them for assistance with that.
    2.  If you own your modem/router and the ISP only provides the signal into your equipment, issue a Google search for the model name of your router and look for 'hard reset'.  That should walk you through finding the reset button and your initial setup again.
